Phase One – Define The Problem
Collect all available data. Get the facts, all the facts. Do not make assumptions, draw
conclusions, or add meaning without first gathering all available data and facts that relate to
the possible cause of the problem. Involve others in decision making.
Phase Two – Assess The Problem
Select the appropriate data, add meaning to that data, and begin to make assumptions and draw conclusions. Brain Storming can be a useful tool in helping to identify the root causes of our problem. Again, it is imperative that we are targeting the true causes of our problem and not merely treating the annoying symptoms of the problem.
Phase Three – Find The Solution
Target root causes with possible solutions. Once a number of possible solutions have been identified, we would next need to prioritize these solutions
based upon their effectiveness. We want to ensure that our time and effort goes into the
solution that will produce the appropriate change. We must also consider possible “side
effects”, or unintended consequences of our solution. Some changes can produce a ripple
effect throughout the organization that can affect morale or productivity negatively. We want
also to guard against the quick fix (putting the bucket under the leak) just because it is easy
or inexpensive to implement.
Phase Four – Plan The Solution
Define what steps need to be taken, who will complete each step, and when will it be completed.
Phase Five – Implementation and Evaluation
Now we need to see what needs to be done to implement our action plan. Do we need
involve others in the implementation of our solution? Have we done our homework to ensure our actions will
not negatively impact others? Are any changes needed in support systems in place that will
allow our solution to occur? All of these items need to be carefully considered in our
implementation phase.
Once implemented, we will need some method by which we can measure the success of our
solution. By carefully monitoring the results of our solution, we can determine if our solution is
having the desired effect.
